General Requirements

The following requirements must be submitted:

1. Passing Result of the Entrance Exam
2. Photocopy of Authenticated Birth Certificate from NSO
3. Photocopy of Baptismal Certificate – (for Catholics)
4. Photocopy of Certificate of Dedication – (for Non-Catholics)
5. Report card
6. Dental Health Certificate – (for Grade 1 only)
7. Good Moral Certificate
8. For foreigners, secure a Permit To Study from the Immigration Office and photocopies of pupil's visa and passport
